You probably spawned around ~4 or ~5 O'Clock position, and you need to go to ~10 O'clock by heading north when possible, and left.
Tl dr, if you end up facing toward the bottom left of your screen when you port into the elder sanctum (floor #2 in the ruins), reference a dial clock.
Throne room is (almost?) always at the far WNW (West-Northwest) of the Elder sanctum. If you end up in the cube room, head left some more. Then just keep going left, and north when you can.Įventually you will either run into the throne room, if the pathing was mostly going left/west Īlternatively, you will run into the room where you originally obtained the 'item' Kanai's Cube from Kulle's quest, if you went north early on before heading too far left. So you'll go southwest from the instance entrance, probably through some straight corridors with guillotines and traps, just keep going until you can go north (top of your monitor.
